SOP: Off-Campus Use Agreement

Effective - May 17, 2007

Purpose: To establish a procedure for off-campus use agreements for ITS.

Applicability: ALL ITS STAFF

1. The forms will be emailed to the group manager from Administrative Services (Rebecca Molinary, Melissa Harris). Each group manager will be expected to deliver the forms to their respective employees. The agreement is only good for one year, beginning on July 1st and ending on June 30th. A new form will have to be filled out annually. Administrative Services will send out an annual renewal notification in June.

Operating Rules:

1. For new employees, the manager can find the Off Campus Use Agreement form under "Forms & Documents", "Human Resources Forms" on the ITS website: http://its.unc.eduforms/index.php.

2. New employees should have this form (if applicable) completed and return to Rebecca Molinary or Melissa Harris prior to taking equipment off campus.

3. Once the employee returns the completed form to their manager, it is the manager's responsibility to get their respective AVC's signature and then return the original forms to ITS Franklin, CB#1150. The only signatures needed on the form are the employee's (next to User) and the AVC's (next to Department Head).

4. When an employee turns in their equipment, for any reason, a Movable Equipment - Notice of Disposal or Change in Location Form (E/S - 102) will need to be filled out and the manager will need to forward that form to Rebecca Molinary or Melissa Harris as well. This form can also be found on the ITS website under "Forms & Documents", "Procurement Forms": http://its.unc.eduforms/index.php

5. The information on all forms will be kept on a spreadsheet. The hard-copy forms will also be kept in a file cabinet in the supply room.
